Medical Records Transfer to New GP Practice
If you would like us to transfer your medical records to your new GP practice, please follow the steps outlined below:
- Complete the Request Form: You must complete and sign a Release of Information form at your new GP practice, authorising us to send your medical records to your new GP practice.
- Submit the Form: After signing the form, please ask them to email it to us at efmc@edinburghmedical.com.au. We will then contact your new GP practice directly to facilitate the transfer.
- Transfer Fees: Please note that a fee may apply for the transfer of your records, based on the size and complexity of your file. Any applicable fees will be communicated to you prior to processing the transfer.
- Transfer Process: Once the request is processed and the applicable fees are paid, your records will be securely sent to your new GP practice.
Please note: This fee is a non-Medicare charge and cannot be claimed through Medicare.

Request for Medical Records from Previous GP Policy
If you would like us to obtain your medical records from your previous GP practice, please follow these steps:
- Complete the Transfer Request: You must sign a Release of Information form authorising us to request your medical records from your former GP practice.
- Submit the Form: After signing the form, we will then contact your previous GP practice directly to request your records.
- Fees: Please note that there may be a fee associated with obtaining your records, depending on the practice policy. Please check with your GP if any fees involved.
- Receiving Your Records: Once we receive your records, your records will be securely stored at our practice.
